Andy Kershaw, Johnny Marsters and Ben Broster come in for Steven Sparks, Joe Ross and Emiliano Bergamaschi at loosehead prop, hooker and tighthead prop respectively, while the pack behind them remains unchanged.
In the backline, Adryan Winnan and Darragh O'Mahony come in for the injured pair of Thomas Castaignède and Nathan McAvoy.
"This is the first time we've really made any significant changes to the side this season and it's not something we've done lightly," said Kafer of the changes.
"Obviously we've been forced into a couple of changes with the loss of Thomas [Castaignède] and Nathan [McAvoy], but with the others we've taken the chance to try some different combinations in order to stabilise our set-piece."
